The Community Navigation service (also referred to as social prescribing) is for young people aged 13-25 who are experiencing emotional health and wellbeing issues (eg. feeling stressed, worried or not enjoying things). You may recognise that you are isolating yourself from others and that you would like to access support, but that you’re not sure how to, or where to start.
We can meet with you for an informal chat and take time to listen and understand your needs. Together we can agree a well-being plan that focuses on your priorities and connect you with activities, events and organisations that can help you feel better.
Taking part in sport, creative arts, volunteering or peer support groups are all examples of activities that would form part of your well-being plan.
Community Navigation is more than a signposting service and our support doesn’t end there. We can arrange follow up sessions to see how you are getting on and provide additional support if necessary.
