Children's Continence Service

Wetting and soiling are among the commonest and most distressing conditions affecting children, young people, and their families. The aim of Southampton Children’s Continence Service is to provide a locality based multidisciplinary assessment, diagnosis, treatment and support for these families. When youngsters come to the clinic they will be seen by a specialist nurse or doctor who will complete an assessment and put a tailored care plan in place. Children will have regular follow up in clinic with frequent telephone support where necessary. Parents can contact the Children’s Continence service hotline to speak to a specialist nurse for information and advice.

What does your service do?

The Children’s Continence Service promotes continence and independence in continence issues in children 2-16 years old, or 2-19 years old if the child has special needs and is in full time education. Our aim is to promote continence where possible with children and young people.  We support the child and their family to live as independently as possible with their continence difficulties.

We provide support for a range of continence issues including: day and night wetting, urgency and frequency of micturition, constipation and soiling, smearing and behavioural difficulties in children with special needs or physical difficulties, and help with toilet training and containment products.

A variety of clinics are offered. We work with parent or carer and the child to actively support and promote continence and treatment. Parents and the children are encouraged to participate in the process by giving feedback at regular points during service delivery.

Where is it located and what areas does it cover?

The clinics are locality based and they are all wheelchair and pushchair accessible. They are held in Bitterne Health Centre, Lordshill Health Centre and Adelaide Health Centre.

We offer support to children within Southampton City and who are registered with a City GP.

Who does your service provide for?

The service is for children 2-16 year old children and children 2-19 year olds who have Special and needs and are in full time education. We do not see children who have neuropathic bladders or bowels as these children are seen in the hospital and require more specialist investigation and treatment.

How can I start using the service?

Written referrals are accepted from GP’s, School Nurses, Health Visitors, Social Care, Community Paediatricians, and acute Paediatricians.

The service is run by Solent NHS Trust and there is no charge for the service users.

The service adheres to Trust guidance of seeing patients within 11 weeks. On receipt of the referral information is sent to clients requesting that they complete an input /output chart or a bowel movement chart as required to complete prior to their appointment. This is an essential part of the assessment process.

How are decisions made about who can use your service?

  • Every referral received is triaged by an experienced specialist continence nurse.
  • The referral is allocated to a locality based clinic near to where the child lives with the most appropriate clinician to meet the child’s needs.
  • The age of the child, special needs, behavioural difficulties and   containment product requirement are considered.
  • If a referral is refused the referrer is contacted in writing to explain why and to signpost to the most appropriate service or strategy to meet the child’s needs.

How do you communicate with service users and how are they involved in decision making/planning?

  • The service actively seeks the client’s thoughts and comments about their needs and treatment plan and possible outcomes.
  • We use the Solent NHS Trust Family and Friends survey to receive feedback on our service and staff. If the child is old enough they are also invited to offer their views.
  • Parents / guardians or carers and children are included in care planning.
  • If English is not the first language we use the interpreter service and offer a longer appointment to make sure understanding is achieved.
  • The parents and children are asked to keep records of their continence issues and their progress is discussed at each appointment. If they are doing well this is recognised with achievement certificates for them. The whole process of achieving continence is about educating parents and child to be able to understand how to become continent and how the body works.
  • For those children with learning difficulties and communication problems we use Makaton symbols for them to identify with.

Is your service fully accessible?

All of the health centres are wheel chair / pushchair accessible.

There are disabled and baby changing facilities.

There are some toys in the waiting rooms, but our aim is not to keep families waiting.

We have some children who do not get anything from the appointment but can still achieve toilet training with parental and school help. In these cases the parent is invited to attend a session without the child. Keeping the child in their usual routine and advice is given to the parent / carer. School are contacted to assist in implementing the care plan.

What training are the staff supporting children and young people with SEND had or are having?

There is a competency matrix that is monitored and training supports this in the service.

Most of the nurses are Paediatric trained and are specialists in child continence care. All staff undertakes mandatory training to keep up to date with health and safety and safeguarding conflict training fire safety etc. This is in line with Solent NHS Trust policy.

We work closely with School nurses, safeguarding team, social care and health visitors to achieve the best possible outcome for each individual child.

How will I know my child will be safe using this service?

All staff are checked by the disclosure barring service (DBS).

The nurses have current registration to practice with the Nursing and Midwifery Council (NMC).

The children are never left alone with a clinician they always have their parent /carer with them.

The Trust is inspected by CQC.

All staff have mandatory training. Part of this training is ensuring there are no hazards in the clinical areas where the children are seen.

All staff have their credentials checked by the Trust when employment is offered.

The clinical areas are secured with digital / electrical locks.

All staff adhere to safeguarding and health and safety policy and guidance.

How do I give feedback, make comments or raise concerns?

We deliver the Solent NHS Trust Family and Friends Survey. This gives parents and children the opportunity to comment on the service and staff. The information from those forms is collated by the trust.

Who can I contact for further information?

At the point of triage the patient is allocated a nurse / clinician with the appropriate skills to meet that patients treatment needs.

They are given the service contact details and told who to phone.

They are also given a generic e-mail address for contact if preferred.

They are invited to phone for advice if they have any concerns or worries. If their allocated nurse clinician is not available to speak to, another nurse will be able to communicate to them.

They can talk to the referrer regarding the benefit of being referred to the service, or they can phone the service and discuss it with one of the nurses or clinicians.

The team Lead is always available and willing to see or speak to parents about any aspect of their treatment and care journey.
