The Community Learning Disability Teams (CLDTs) are based across Hampshire and Southampton and provide specialist health assessments and interventions for people with a Learning Disability to identify and address unmet health needs.
The CLDTs work with adults aged 18 years and over. In addition, the team will work with young people approaching their 18th birthday, where indicated through a formal transition plan.
Eligible service users will require significant support to manage their health, and have unmet needs which require specialist input, that cannot be met by generic health services, even with reasonable adjustments.
The service is delivered in line with Southern Health’s seven Clinical Areas of Practice. Service users will have significant needs relating to one or more of:
- Challenging Behaviour
- Mental Health
- Complex Physical Health
The teams comprise of:
- Community Learning Disability Nursing
- Speech and Language Therapy
- Occupational Therapy
- Art Therapy
- Administration staff
- Managers and Learning Disability Strategic Health Facilitators
