We support for 16-25 year olds that have epilepsy to help them get into training, employment, education or voluntary work. We have a holistic approach and strive to achieve a well rounded quality of life, sense of purpose and network of friends.

The Transition support worker is based in Southampton and travels to meet the young person wherever they are in Hampshire. We can meet at home or in the community, please note that our postal address is outside of Hampshire but the project is based in Southampton.

Young Epilepsy Transition Support is a free project working with 16-25 year olds, with epilepsy living in Hampshire. Whilst young people with epilepsy have support from their health professionals, we recognise that other areas of their lives may be impacted quite severely, such as housing, independence, developing their personal, social skills and feeling very isolated. We encourage young people to have a sense of purpose, gain a network of friends and to live a well rounded life. The Transition Support Project works with young people to support them to identify and overcome issues or barriers to further education, training, employment or any other areas of their lives. The project also works alongside other organisations, to support them by supporting your young people such as at appointments, activities, volunteering and employment.
