This assessment is an opportunity for you to look at how caring may be affecting your life. It looks at various areas of your wellbeing including your physical, mental and emotional health.

You can tell us what's important to you, what you value about your caring role as well as any changes that could improve your own wellbeing and quality of life.

To complete this self-assessment, you must be over 18 and provide care and support to an adult (also over 18). If you are a young carer (under 18), or a parent carer, please use the information links to the right of this page for advice and guidance.
